The idea that an 80-year-old might have terminal cancer is hardly a surprise, so this is not all that dramatic an idea–especially since we know that Castro had serious surgery back in the summer. Further, it is not unreasonable to assume that the Cuban government has downplayed the seriousness of that surgery.
Given Castro’s age, the likelihood of his death, and thereby a regime transitions, cancer or not, grows daily.

Hasn’t the transition already occured? Raul is still effectively head of state.

To be honest, I question the degree to which Raul is in charge. Yes, that’s the official word, but I don’t really take the Cuban government’s word for what is actually going on.
And the real transition won’t start until Fidel dies.
